To ensure new patient referrals are processed quickly, please see our Referral Process Form for a summary of all documentation that should accompany new requests faxed to our confidential referral line: 312-277-9575. After receiving a referral (during standard business hours) we will check eligibility, verify insurance benefits, and assist with prior authorization requirements by the end of the business day. Once benefits are confirmed we will coordinate free next day delivery service with the patient and home health nurse. We never deliver medications without full disclosure of out-of-pocket expenses, and offer financial assistance sourcing support from licensed Medical Social Workers to patients who cannot afford prescribed drugs. For physicians who provide infusion therapy to their commercial insurance patients in their office, we offer free shipping.
